My nephews that my oldest brother had are all Fragile-X Downs Syndrome. I grew up taking care of them and my other niece and nephews for many years. It's all about finding their happy place when things go wrong (and they will.)
The most important part about being a nanny...
The best moment was when my niece (who was about 5 at the time) was yelling for someone to help wipe her, I guess I was taking too long because maybe 2 minutes later she just yells "I got it, thaaaanks!"
